Application Checklist
READ FIRST – This checklist will assist you in keeping track of the necessary items that you must complete for the application process.

Candidate Selection Form
The program bases selection on a point system. This form will allow you to track your accumulated points.

Pre-admission Testing Required – The following tests must be completed by the application deadline.  OTC Testing Services will proctor both exams.  You must register with Testing Services before testing.  Click here to access information concerning these tests and registration.

Application Form
Refer to the application form for directions.

Reference Form
Print three references and distribute them to individuals who are able to comment on your academic potential, work performance or potential to function in a healthcare field.  ALL REFERENCES MUST BE RECEIVED BY THE APPLICATION DEADLINE.

Job Shadowing/Observation Form
Applicants are encouraged to spend 3 hours in a surgical setting at a hospital of your choice.  This is not required but recommended.  Points will be given as part of the admission process.

Wonderlic Sample Test Questions

Sample Math Test
Students who are invited to the interview will be completing a math test as part of this process.  This sample test will give you an indication on what to expect.

Immunization Requirements
Students will be requested at the time of acceptance to submit documentation for required immunizations, TB testing, and current Basic Life Support for Healthcare Providers CPR.

OTC cannot guarantee that students classified under the Deferred Action for Childhood Arrivals (DACA) program will receive approval to take the licensure exam after program completion. Please contact the state or national agency that administers the professional licensure exam to inquire about licensure exam requirements.
