Off-Campus/Dual Credit Proctoring Approval Form
(2 Business Days Required for Processing)
- Student Instructions:
- Students must submit a separate form for each proctored exam they wish to complete at a non-OTC facility.
- Before you submit the Off-Campus/Dual Credit Proctoring Approval Form, you will need to reach out to your non-OTC facility to see if they are available to proctor your exam.
- For dual-credit students: The best and preferred option is for students to take their proctored exams at their high school with a high school employee designated as the proctor.
- Once the form is submitted, it will not be approved until Testing Services contacts the non-OTC facility to see if they will adhere to the following proctoring expectations:
- Verify the student’s identity.
- Ensure the testing room is quiet and conducive to testing.
- Monitor the student for the entire duration of the exam.
- Ensure the exam is completed in one sitting.
- Do not allow the student to see or enter the password.
- Follow all instructions provided by the instructor.
- For paper/pencil exams, scan and email the completed exam to the instructor.
- Report any incidents of suspicious behavior or blatant cheating to the instructor.
- Proctors should not be relatives, friends, clergy, or employers of the student.
- Once Testing Services hears back from the non-OTC facility, an approval or denial email will be sent back to the student.
- If approved, Testing Services will send the exam and exam information the day before you are to test to your non-OTC facility.
- If denied, the student will need to find a different location and resubmit the Off-Campus/Dual Credit Proctoring Approval Form.
- This is why we request at least 2 Business Days for processing.
