The Faculty Request for Make-Up Exam form is for faculty members to make arrangements with their students to take the exam before or after the scheduled exam date.
Students will need to be in contact with their faculty instructor for approval to take a Make-Up Exam. Students taking a Make-Up Exam will also need to schedule a time for their exam with Testing Services.
