Application Checklist<\/a><\/p>\n

READ FIRST<\/strong> – This checklist will assist you in keeping track of the necessary items that you must complete for the online application process.<\/p>\n

Work Verification Form<\/a><\/p>\n

If you are applying to the Springfield Advanced Hybrid Track PN program, you must be a CNA, MA, or EMT with at least 6 months working experience in the past 2 years. Complete this form and upload it when prompted during the online application process.<\/p>\n

Immunization Requirements<\/a><\/p>\n

Students will be requested at the time of acceptance to submit documentation for required immunizations, TB testing and current Basic Life Support for Healthcare Providers CPR. The COVID vaccine is required by both Cox and Mercy Hospitals. Those hospitals are the primary clinical sites for all three Practical Nursing Programs.<\/p>\n
