Surgical Technology Admission Information – Application Deadline: March 15 (Program begins in the fall)
OTC cannot guarantee that students classified under the Deferred Action for Childhood Arrivals (DACA) program will receive approval to take the licensure exam after program completion. Please contact the state or national agency that administers the professional licensure exam to inquire about licensure exam requirements.

Candidate Selection Form
The program bases selection on a point system. This form will allow you to track your accumulated points.
Preadmission Testing (Bennett & Wonderlic) – Students must complete this testing by the application deadline. Click here to access information concerning the Bennett & Wonderlic preadmission exams. OTC Testing Services will proctor this exam.

Print three references and distribute them to individuals who are able to comment on your academic potential, work performance or potential to function in a healthcare field. ALL REFERENCES MUST BE RECEIVED BY THE APPLICATION DEADLINE.
Job Shadowing/Observation Form
Applicants are encouraged to spend 3 hours in a surgical setting at a hospital of your choice. This is not required but recommended. Points will be given as part of the admission process.
Sample Math Test
OTC will schedule a math test after the application deadline for those who meet the minimum requirements. The testing center will contact you after you’ve scheduled your appointment.
