To obtain an initial license in Missouri, all levels of EMT must provide proof of current certification with the National Registry of Emergency Technicians along with page one of the EMS personnel license application. Click HERE for additional information.
Below you will find the necessary forms to apply to the Paramedic program. The required forms must be received in the Health Sciences Office by the application deadline to be considered as a potential candidate.

Paramedic Reference Form
Print three references and distribute to individuals who are able to comment on your academic potential, work performance or potential to function in a healthcare field. ALL REFERENCES MUST BE RECEIVED BY THE APPLICATION DEADLINE.
Paramedic Candidate Selection Form
Selection into the program is based on a point system. This form will allow you to track your accumulated points.
Students will be requested at the time of acceptance to submit documentation for required immunizations, TB testing and current Basic Life Support for Healthcare Providers CPR.
OTC cannot guarantee that students classified under the Deferred Action for Childhood Arrivals (DACA) program will receive approval to take the licensure exam after program completion. Please contact the state or national agency that administers the professional licensure exam to inquire about licensure exam requirements.
