Off-Campus Proctoring Approval Form

(2 Business Days Required for Processing)

  • Student Instructions:
    • Students must submit a separate form for each proctored exam they wish to complete at a non-OTC facility.
    • Before you submit the Off-Campus Proctoring Approval Form, you will need to reach out to your non-OTC facility to see if they are available to proctor your exam.
    • Once the form is submitted, it will not be approved until Testing Services contacts the non-OTC facility to see if they will adhere to the following proctoring guidelines:
      • The identity of the student is to be verified.
      • The student is monitored during the entirety of the exam.
      • The exam must be completed in one sitting.
      • At no time will the student be allowed to see a provided password.
      • If this is a paper/pencil exam, you (the proctor) will be able to scan and email the exam back to the instructor.
      • You (the proctor) are not a relative, friend, clergy, and/or employer.
    • Once Testing Services hears back from the non-OTC facility, an approval or denial email will be sent back to the student.
      • If approved, Testing Services will send the exam and exam information the day before you are to test to your non-OTC facility.
      • If denied, the student will need to find a different location and resubmit the Off-Campus Proctoring Approval Form.
      • This is why we request at least 2 Business Days for processing.

  • Important limitations:
    • The below form is NOT to be used for COM-100/105 proctoring. Instead, please visit the Speech Communication Center for off-campus proctoring requests.
    • If you attend a OTC Dual Credit program high school, do NOT submit this form. Contact your designated high school proctor for instructions.

  • Example of Approved Proctors:
    • University testing center administrators, high school administrators, professional librarians at a public library, testing control or unit education officers at U.S. military bases, or U.S. embassy officials.

  • Example of Unapproved Proctors:
    • Self, relatives, friends, clergy, and employers.
